When you get to the site you may have trouble getting the form to load without disabling Javascript.
For CHROME, when you get to the site, right click anywhere on the screen and click “view page info”. Then scroll down to Javascript and click “block”. Come back here and click the link in my post again.
Don't forget to enable Javascript again thought when you are done filling in your form. You should receive your sample in 4-6 weeks!
